Request in Zendesk - 

Please create a  Azure SQL server and portal-db under this server with the following details.

Subscription : SIRVA NA

Resource group : EDM-PRD

Server name : sql-cac-ms-pr-01

Location : Canada Central

Database name : portal-db

Authentication : SQL Server authentication

Server admin login : WebOpsMTS

Password : 2dayisaG8TdA

Also, grant access to the following Waii team members (please include their IP addresses under the Networking section):
